Job Summary
Responsible for performing the new patient intake procedures and identifying appropriate candidates for home care services. Evaluates patients referred for treatment to determine appropriateness for admission.
Job Responsibilities
- Obtains all patient information, demographics, insurance, or medical Information to complete the referral.
- Obtains initial and ongoing insurance/managed care benefits for insurance providers.
- Ensures prompt admission of patients and obtains medical records in a timely manner.
- Initiates welcome call to the patient or patient representative after the intake process and a clinician has been assigned.
- Assists patient and patient representative with any questions they may have.
- Maintains and updates database and spreadsheets with current data, pertinent information, and status of the admission referral as appropriate.
- Scans patient’s documentation daily into the electronic medical record (EMR) and corrects patient chart with correct naming conventions.
- Provides care team with reports regarding pending admission, new admission, and outstanding (open) items.
- Collaborates with staff and management to effectively resolve issues impacting a patient’s admission process.
- Escalates admission denials to leadership for further action.
- Complies with all local, state, and federal regulations.
- Assists with the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
- Maintains positive working relationships with current and potential referral sources.
Qualifications
- High school diploma, General Equivalency Diploma (GED) or higher required upon hire
- A minimum of 2 years' relevant experience required
- Current licensure or certification in clinical specialty required upon hire AND
- Driver - Valid driver's license and 21 years of age required upon hire
