As a Community Case Manager for Living Innovations, you will be responsible for supporting individuals to have a good life at home and in the community. Case Managers at Living Innovations assist by providing coordination and oversight during the planning and delivery of Maine home and community-based waiver services. The ideal candidate will be able to communicate effectively in writing and verbally with coworkers, family members, guardians, and individuals at all levels of the organization. To ensure quality service delivery, all candidates must be able to manage their time and resources efficiently and effectively. Remote flexibility availability after first 90 days.
Must have BA and two years experience with the IDD populations to be considered.
Responsibilities
· Facilitating communication between members of the support team (individual, guardian, family, service providers and others as appropriate)
· Working with the support team to develop a person-centered plan.
· Linkage to appropriate agencies, community resources and informal supports.
· Monitoring the individual’s progress toward the achievement of objectives specified in the plan.
· Periodic review of the individual’s status and needs and implementing changes to the plan as needed.
Qualifications
· Bachelor's degree.
· Two years’ experience with IDD/Autism is required.
· Valid state issued driver’s license, current vehicle insurance declaration and vehicle registration.
· Must pass comprehensive background checks per state and company regulations/policies.
