Job Overview
Working with our community partners, CCEH's mission is to end homelessness in our community. We are currently seeking a full-time Targeted Case Manager. You'll be a part of the CCEH team as we maintain a safe and welcoming environment for our guests and work cooperatively towards our mission. Come grow with us!

The Targeted Case Manager is responsible for working with individual Resource Center clients on a walk-in basis during RC open hours (9am-11:30am) and by appointment in the afternoon. The case manager collaborates with staff from CCEH's Supported Housing program, Emergency Shelter and other local agencies conducting outreach and/or providing services to the homeless.

Duties

• Meet individually with Resource Center clients for welcome, orientation, assessments, data collection and case management.
• Help clients connect with mainstream community services and benefits.
• In conjunction with the Resource Center team, supervise clients and monitor overall dynamics, dealing with any conflicts, or behavioral/medical issues..
• Track daily client data through Salesforce and HMIS.
• Work collaboratively with CCEH's Housing First Program and Emergency Winter Shelter (on site), to identify eligible participants, provide supplemental information and provide support to clients as appropriate.
• Develop and maintain relationships with the wider homeless services community, such as local churches, city welfare, Concord PD social worker, Catholic Charities, etc....
• Participate in regular supervisory and staff meetings.
• Represent CCEH within the community at various events.

Skills

We are looking for people with integrity, good interpersonal relationship skills, and who are team oriented. You must be able to remain cool under pressure, be good at problem solving, and a critical thinker. You must be able to remain flexible, yet decisive and know when to "shift gears." You'll need to possess great communication skills, both in person and written, be organized and be efficient in your time management.

Job Type: Full-time

Pay: $20.00 per hour

Expected hours: 40 per week
