We are looking for a Registered Nurse (RN) to join a dynamic team.  This role will be responsible for providing skilled nursing care to homecare patients.

Responsibilities

Here’s how you’ll move healthcare forward in this role:

• Take part in a two-week clinical orientation to learn key procedures, the EMR system, compassionate care, and safety.
• Assess and evaluate the needs of patients with multiple disease processes and comorbidities and develop an appropriate plan of care
• Provides education and strategies for patients and families
• Partners with other VNA clinicians and external providers to execute best patient outcomes
• Documents all changes in patient’s condition and updates care plans and outcomes

Qualifications

The ideal qualifications for the Registered Nurse, Homecare, include:

•  Active Registered Nurse license. BSN a plus.
• 1  year of supervised clinical experience as a registered nurse, preferably in home health or medsurg
• Excellent verbal and written communication skills. Solid interpersonal and time management skills
• Strong computer skills
• Strong problem solving skills
• Car and valid driver’s license

Working Conditions/Physical Demand: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact
