The Home Health and Hospice Nurse Administrator will oversee a growing and busy mission driven service provider in the area. The Home Health and Palliative Care Administrative Director will have the scope of responsibilities including Home Health, Hospice, HME/HIT and Private Duty. 

As part of the health system initiatives, the hospital is looking to expand and grow the services and looking for the right individual to lead in this endeavor. The Home Health and Hospice Nurse Director will report to the Chief Operating Officer and will oversee Physical Therapists and nursing staff to ensure patient care, infection prevention and best medical practices for the hospital. 

The Home Health and Hospice Nurse Director will manage and enhance community outreach to strengthen the network of outpatient referrals and admissions while developing and fostering positive physician relationships. The Home Health and Hospice Nursing Director will work collaboratively with the executive team and provide leadership in care coordination to medical staff.
