Instructor - Educational Assessment jobs in Albany, GA

B&B Care Services, Inc. is looking for an Assessment Nurse to join our team. This position is a remote position that requires routine travel.

Responsibilities:

• Assess and document the physical and psychological health of residents in accordance with state and federal regulations under the Elderly and Disabled Waiver Program (EDWP).
• Complete all assessments and care plans as identified by services requested.
• Complete all documentation within established Standards of Promptness.
• Participate in the orientation of new personnel, in-service training programs, case conferences and staff development meetings.
• Ensure that patients and caregivers understand the patient's rights and responsibilities.
• Utilize time management techniques when scheduling and planning daily work activities.
• Review initial financial, medical, and social information of potential client as presented by referral source or the applicant.
• Verify DFCS Medicaid eligibility and/or screens for MAO/PMAO eligibility for using a standardized guideline.
• Perform other related duties as assigned.

Education & Experience:

• Graduate of an approved school of nursing with a current state nursing license in good standing without state or federal violations.
• Two years experience as a RN or LPN in one of the following areas of practice:
• Geriatric Nursing
• Community Health
• Long Term Care
• Chronic Diseases of Adults
• Current certification in CPR and First Aid.
• Reliable transportation with current drivers' license and proof of liability insurance.
• Satisfactorily complete a criminal background check and drug screening.

Job Types: Full-time, Travel nursing

Pay: $23.00 - $28.00 per hour

Expected hours: 40 per week
