Full Time

Position available for Assessment Nurse to provide medical assessment to elderly and adults with physical disabilities in Forsyth County as part of a multi-disciplinary team. Assessment and reassessment completed as indicated by policy and funding requirements.

Education, Experience, Training:

Nurse who holds a current North Carolina nursing license. Experience in geriatrics, home health, long term care, or CAP/DA preferred.

Knowledge, Skills, Ability:

• Basic competency in Microsoft Word and Excel
• Understanding and sensitivity to diverse backgrounds and lifestyles
• Knowledge of community resources
• Medical history assessment skills
• Ability to articulate ideas clearly, (both written and orally), listen, and establish personal rapport
• Good organizational and documentation skills
