Position Purpose:

The “Medication Reconciliation” Pharmacy Technician will primarily be responsible for obtaining a complete and accurate personal medication history, including allergies and other intolerances, for patients that are admitted or still in the ED and are likely to be admitted to the facility. The goal will be to complete the list as early in the patient’s stay as possible and will consist of as much ‘direct patient interaction’ as is practicable. However, other resources, (ie patient family members, caregivers, medication lists provided by the patient, outside pharmacies, prescription bottles, past medical history, medical clinics, and other healthcare
providers) may be consulted in order to facilitate completion of the “Home Medication List”.  This person will be responsible for communicating relevant information to facility providers who are (or will be) involved in the patient’s care. (ie. Notification of the Home Medication List ‘readiness’ status, pending clarifications, etc.)

This person will also have responsibility for all “quality assurance” activities and unit inspections for the Emergency Department.

This employee may also be called upon to perform other duties typical to those of a “Registered Pharmacy Technician”.
