    Case Care Manager

    $37,000/yr - $44,000/yr · Salary - Full-time position

    Position Summary:

    We are seeking a Case Care Manager to join our home care team. As a Case Care Manager, you will coordinate caregiving services to assigned Clients. You will develop individualized care plans for your clients with the end goal of ensuring a healthy & positive in-home lifestyle supported by our caregivers that empowers client independence. Leading recruitment efforts, you will interview caregivers for current and new cases, set and manage the schedules, follow-up with caregivers, and manage office administration tasks.

    Position is 40-45 hours a week average - 8AM to 5PM Monday through Friday - Some On-Call after Hours required.

    Key Duties & Responsibilities:

    • Answer the main office phone during normal operating hours. On-call off hours on flexible rotation.
    • Lead and manage recruitment initiatives, including calling leads, conducting interviews, hosting orientations, acquiring necessary documentation, and sending & confirming employee paperwork.
    • Schedule and coordinate case shifts for clients and caregivers.
    • Conducts screening and assessments to develop a comprehensive and individualized plan of care.
    • Follow up with clients and families to ensure quality of service.
    • Collaborates with co-workers and caregivers to allow for patients to overcome obstacles through impactful care coordination.
    • Confirm accuracy of completed shifts for billing & payroll.
    • Participate in team meetings and quality improvement initiatives.

    Qualifications:

    Education:

    • Associates degree (required); Bachelor's degree (preferred) - Education may be supplemented with additional relevant experience.

    Experience:

    • Knowledge of customer service principles and practices (required).
    • Care Coordination or similar quick to action account management (required).
    • Scheduling and shift management (required).
    • Employee recruitment, orientations, and hiring paperwork (required).
    • Basic computer competency - including Windows Operating System, all Microsoft Office programs, Google Suite, and Zoom (required).
    • Knowledge of WellSky Home Health Software, PandaDoc online documentation system, & GoTo phone and meeting system (preferred).

    Work Environment:

    The Case Care Manager is a hybrid role, 2 to 3 days in-office at our Farmington Hills offices, 3 days remote. The Case Care Manager must be available and willing to be on-call in a rotation. This position may require occasional commutes in the Metro Detroit region on an on need basis for case assessments.
