Under the supervision of the RSVP Program Director, the Intake Specialist/Transportation Coordinator plays a crucial role in ensuring that clients receive timely and effective services. This position involves conducting initial client intake assessments, maintaining accurate records, and coordinating transportation services. The ideal candidate will be detail-oriented, empathetic, and capable of managing multiple responsibilities to ensure seamless service delivery.
· Welcome clients to Nevada Rural Counties RSVP Program, Inc. by initial phone call.
· Conduct initial intake interviews with potential clients, assessing their needs and eligibility for services.
· Explain available services and programs, answering any questions clients may have.
· Complete client application paperwork and enter it digitally for new and existing clients.
· Schedule client's initial home visit
· Answer phone and email inquiries.
· Address any client complaints and concerns.
· Perform clerical work such as data entry, filing, copying,
· Enter client case notes digitally.
· Coordinate client relations with the County Service Coordinator
· Answer phone and schedule transportation requests
· Conduct client home visits in Carson City and the surrounding area.
· Gather and maintain accurate, up-to-date information, ensuring all required documentation is completed and filed appropriately.
· Coordinate with other departments to ensure clients are matched with appropriate services and support.
· Follow up with clients as necessary to ensure their needs are being met and update records accordingly.
· Provide clients with information on resources, services, and community programs they may benefit from.
OTHER
· Perform other duties and/or projects as assigned.
· Schedule and coordinate transportation for clients to medical appointments, grocery shopping, and other essential services.
· Communicate transportation schedules to both clients and drivers, ensuring clarity and timely services.
· Work with the transportation team to monitor and manage trip logs, routes, and schedules.
· Resolve transportation-related issues, including last-minute cancellations or scheduling changes, and communicate updates to all parties involved.
· Ensure compliance with company policies, safety standards, and regulatory requirements for transportation services.
· Track and report transportation usage and costs, identifying opportunities for improving efficiency and service delivery.
