Priority Healthcare Group is seeking a Clinical Liaison for our Skilled Nursing Facility clients (Forest City, PA).

The Clinical Liaison is responsible for managing patient referrals in order to ensure a smooth transition.

Responsibilities:

·       Clinical assessment of electronic medical charts from hospitals to determine if a facility can meet the clinical needs.

·       Verification of benefits

·       Obtain pre-authorizations from insurance company

·       Communicate clinical information to facility

·       Ensure closure with each referral

Qualifications:

·       RN/LPN/LSW, or

·       Over 2 years' experience in nursing home industry in one of the following roles: Central admissions, Director of Admissions, Clinical Liaison, Director of Marketing, or Social Services
