Licensed Clinical Social Worker provides clinical social work services to patients and their families. Counsels and provides crisis intervention for patients, including assessment and treatment of emotional and behavioral problems. Being a Licensed Clinical Social Worker coaches patients on how to manage ongoing health conditions and provides therapy to help patients make positive behavior changes. Requires a master's degree in social work. Additionally, Licensed Clinical Social Worker requires Licensed Clinical Social Worker (LCSW). Requires the applicable State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. Position Summary:
Responsible for consultation and direction of social services in the acute and skilled nursing facility. Provides orientation and education to the social service and nursing staff. Develops, organizes, and implements programs, policies and procedures, conducts case work services and counseling and recommends functions necessary to ensure overall operational viability. Maintains performance improvement activities within the department.
Responsible for discharge planning of all patients to include review of social determinants of health. Ability to efficiently conduct plan of care meetings and work closely with utilization review nurse, director of nursing or their designee(s), and healthcare providers.
Education/Experience:
Current State Clinical Social Work Licensure.
Three or more years of previous experience in all aspects of social services and discharge planning.
