SUMMARY:
In keeping with standards recognized by both NCQA Medical Home Recognition and JACHO, SWLA has created teams of professionals to care for the clinical needs of patients who choose SWLA as their primary care provider. The team consists of physicians, mid-level practitioners and the nursing support which is described in this job description. Given the nature of the duties required by the nursing support personnel, SWLA will recruit and hire applicable personnel in the categories of Medical Assistant, Certified and non-certified, and Licensed Practical Nurses. Salary will be adjusted for education, licensing acquisition and experience.

SWLA Center for Health Services (SWLA CHS) has a policy that requires all employees to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they receive an approved exemption based on medical condition, or sincerely held religious belief that prevents them from being vaccinated.
If you are not fully vaccinated, you will be required to have received at least one COVID-19 vaccination prior to your first day of employment and provide proof of your vaccination status on the Orientation Day of your employment. As a part of this process, you will be required to provide information or documentation about the reason you cannot be vaccinated. If your request for an exemption is not approved, you will be required to be fully vaccinated or your employment may be terminated.
