You will provide skilled nursing care to patients in their own environment. Become an integral part of a dynamic, multi-disciplinary team while maintaining a high level of autonomy. Home Care experience beneficial, although we will train the right individual.

Duties Include:

• Provides a complete physical assessment
• Regularly re-evaluates patient nursing needs.
• Follows the developed plan of care which establishes goals, based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Makes necessary revisions as patient status as needs change.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Assessment, Education, and Administration of medications and treatments as prescribed by the physician.
• Provides health care instructions to the patient and families as appropriate per assessment and plan.
• Evaluates for other discipline needs, obtains orders and collaborates on care plan.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
