The Service Coordinator is responsible for coordination of services, and communicating service delivery information to clients and employees in a timely, professional and courteous manner.

Primary Duties and Responsibilities:

• Answers all incoming calls and transfers to the appropriate person.
• Keeps accurate Call Log information for incoming calls and/or Client Waitlist.
• Approves all employee visits in Practice Management program. Obtains paper visit reports (from employees) when necessary.
• Files all company documentation. Files all visit reports and client paperwork in the client chart in the appropriate place in chronological order.
• Responsible for copying client forms, creating open packets, and sending/faxing out informational brochures and employment applications.
• Processes all outgoing mail for the office.
• Processes all Long-Term Care insurance submissions for client reimbursement.
• Completes General Orientation.
• Performs scheduling of all client visits per company scheduling protocol.
• Provide On-Call coverage one weekend per month.

Knowledge, Skills, and Abilities Required

• All candidates must possess at least three years of employment experience in customer service, human relations or a related field, with proven experience managing schedules.
• Candidates must have experience in operating Microsoft Windows and Google doc applications.
• Have the ability to problem solve, work collaboratively with other professionals, family members, and individuals in the community, and accept personal responsibility for maintaining a professional relationship with the company’s clients and the client’s families.
• Must be able to function in an environment with minimal direct supervision.
• Must accept responsibility for learning and adhering to the agency’s policies and procedures on an on-going basis.
• Ability to meet agency’s conditions of employment regarding health clearance and provision of satisfactory work.

A criminal background will be performed on all employees. Employment is contingent upon a clean criminal background.

Scope of Position: Reports to Office Manager.
