This position supports Mercy’s philosophy of patient centered care by assuming an oversight role in managing the services provided in the clinical specialty, participating in staff development, serving as a resource to staff and coordinating chronic care management services with Sedlacek Treatment Center. The Case Manager, Opioid Use Disorders & Substance Use Disorders serves as the first line liaison to referrals with patients needing multiple sources of healthcare support. The role integrates and coordinates resource utilization management, care facilitation and transition of care services planning functions. In addition, the Case Manager – Opioid Sue Disorders & Substance Use Disorders helps drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, education, process improvements, patient satisfaction).
