SUMMARY OF JOB:

Assists in meeting the psychosocial needs of patients/families and enables patients to utilize health care in order to achieve their optimal level of health.

RECRUITMENT REQUIREMENTS:

• Master’s (MSW) degree from an accredited school of social work.
• 1-2 years Post graduate degree experience in a health care setting, (hospital/home care preferred).
• Flexible, sound judgment, customer oriented, organized, keeps professional demeanor at all times.
• Excellent verbal and written communication skills.
• Available to work flexible hours, with weekends and holidays included.
• Knowledge of general disease process.
• Knowledge of family dynamics and interpersonal relationships.
• Able to travel to patient’s home within service area.
• Able to use office equipment; i.e. computer, tablet, cellphone.

LICENSE AND REGULATORY REQUIREMENTS:

• Current license in state of practice required (LSW)
• Valid driver’s license.

CORE RESPONSIBILITIES/ESSENTIAL FUNCTIONS:

• Performs assessment and evaluates individual family environmental needs – emotional, social and financial as related to complicated chronic, high-risk medical problems.
• Assesses and evaluates the coping mechanisms of the patient and family.
• Ability to assist with overall plan of care, integrating short and long term needs with resources.
• Ability to dynamically understand and interpret the regulations defining the delivery of social work services and practice.
• Ability to provide crisis intervention.
• Ability to coordinate services and act as an integral member of the interdisciplinary team.
• Ability to teach and educate patients to access complex social systems and resources.
• Ability to manage, access link and advocate community resources.
• Ability and competence to provide counseling to patients and caregivers.
• Participates in weekly CM review to discuss and address identified high risk patients and/or issues
• Ability to document in a way that reflects understanding of the social work practice and meets regulations.
• Participates in accreditation survey preparedness and related corrective action follow up.
• Is aware of and participates in quality improvement initiatives and infection control plans as necessary.
• Understands and complies with all local, federal and regulatory body requirements.
