Position Summary:

The Social Worker plays a critical role within the interdisciplinary team, performing psychosocial assessments of RMG Lakeside members/ACO beneficiaries and interpreting psychosocial implications of medical issues to case managers and primary care physicians as needed. Must be able to work with Dual eligible population (Medicare/Medi-Cal) as well as Medi-Cal population.

Performs psychosocial assessments, crisis interventions; safety evaluations; work with case managers to assess eligibility for palliative/hospice care; coordinates multiple services to ensure that the psychosocial needs of members/beneficiaries are addressed and provides on-going social worker support based on need. Must be capable of being independent with decision-making within the Scope of Practice and Job Description

In summary, Social Worker is responsible, along with other members of Medical Management Team, for contributing to the clinical, quality, financial and patient satisfaction outcomes of RMG Lakeside.

Essential Duties and Responsibilities:

• Develops individual treatment plans and makes recommendation for members and beneficiaries. Implements treatment plan using appropriate modalities, including individual counseling, medication monitoring, crisis intervention, community contacts, diversion and triage. This can be achieved by meeting with the member /beneficiary and/or responsibly party in the clinic, acute/SNF, or home setting.
• Attend and contributes to Team Conferences regarding care to/for specific member/beneficiary.
• Will respond to requests from Concurrent Review or ACO Case Managers/Vital Care Case Managers to evaluate members/beneficiaries in the skilled setting to who have been determined as potentially unsafe for discharge. Will work with the skilled facility’s Social Worker/designee to provide a safe discharge plan prior to member/beneficiary’s discharge.
• Interprets psychosocial implications of illness to attending staff and departmental staff as needed. Will attend and participate in Department meetings.
• Will consult upon request of clinical and/or educational staff members (within L&AW) and/or accept referral from Case Management or P.C.P. office, for members needing resource information.
• Prepares reports and compiles statistical data regarding members/beneficiaries, as determined by the management team.

South LA, West LA, Downtown LA, South Bay, Saint Francis & North Hollywood

Education and / or Experience:

• Licensed LCSW preferred, with minimum MSW.
• Preference in Bilingual (Spanish speaking) candidates
• High level knowledge of Case Management processes and principles.
• 2 year’s experience in MSO environment.
• Ability to work with all levels of management.
• Must have excellent communications skills both verbally and written.
• Must have excellent organizational skills.
• Required to travel for member appointments.
• Must have working knowledge of MS Office environment, and EZ Cap.
• Requires current CA driver’s license and car insurance

The pay range for this position at commencement of employment is expected to be between ; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

Job Type: Full-time

Pay: $72,000.00 - $75,000.00 per year
