Following a customized care plan, you’ll be assisting, assessing and counseling patients and families on a variety of medical and personal issues. Your job will be to bring your wealth of knowledge to bridge the gap between our clients and the resources they need. This work can be fast-paced and multifaceted, and we’re looking for someone who can handle a variety of situations on a daily basis.
Duties:
· Assist the physician and other members of the home health team to understand the significance of social and emotional factors related to the patient's care
· Perform assessments and reassessments that establish measurable short and long-term therapy, counseling, and community resource planning goals
· Assesses and reassess each patient's psychosocial, social, emotional, spiritual and resource factors to help them and their family cope with daily living issues and to help them follow medical recommendations and the care plan
· Make referrals to other service options and community and family resources to assist the patient in long-term goals or to learn to live within a disability
· Develop plan of care with Registered Nurse case manager, physician, patient and family to deal with personal, financial and environmental difficulties
· Analyze assessments and re-assessments to determine potential and actual problem lists and the need to modify the planned care/interventions
· Develop, prepare and maintain individualized patient care progress records
· Follow all agency policies and procedures regarding service delivery, documentation, and care coordination
· Performs social work assistant supervision
· Assure coordination and continuity of care plan by maintaining liaison with other health care professionals assigned to the same patient, and attend patient care conferences as needed
· Participate in quality assessment and performance improvement programs as needed
· Understand and apply universal precautions and infection control principles to a broad range of clients
Qualifications:
· Master's degree from a school of social work accredited by the Council on Social Work Education
· Minimum of one year of social work experience in a health care setting, with acute care and certified home care experience preferred
· License to practice as a social worker in the State of Michigan
· Current Michigan Operator's license, with reliable transportation and willingness to drive in inclement weather
· Willingness to maintain a flexible schedule and work the required hours, including evening and weekend hours as agreed upon
