Mental Health Case Manager jobs in Chillicothe, OH

We are seeking a dedicated Case Manager to join our team at our Fayette County Clinic. The Case Manager will be responsible for providing Community Psychiatric Support Treatment (CPST) services to adults, children, and their families. These services include: Individual and Group CPST services, Partial Hospitalization Treatment and the coordination, monitoring and overall service delivery internally and with external systems. The Case Manager must determine intensity of medically necessary services. Some services may be provided off site and in the community. The Case Manager will ensure documentation compliance.

Minimum Requirements: Bachelor’s Degree preferred. A High School Diploma with three years related experience and/or training required. Valid Ohio Driver’s license, use of personal car, and annual training standards as required by the Center.
