Minimum Data Set (MDS) Coordinator (LPN) assesses long-term care patient health needs in collaboration with the Minimum Data Set (MDS) Coordinator (RN). Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Minimum Data Set (MDS) Coordinator (LPN)'s years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. MDS Coordinator Duties:
- Completing accurate assessments, MDS & care plans as assigned
- Supporting activities as assigned
- Maintaining all care plans and assessments as required
- Monitoring & auditing clinical records, ensuring accuracy & timeliness
- Informing MDS Coordinator of persistent issues related to non-compliant documentation
- Protecting the confidentiality of Resident & Facility information at all times
MDS Coordinator Requirements:
- MUST HAVE PRIOR MDS 3.0 EXPERIENCE
- Valid New Hampshire Registered Nursing License
- Long Term Care Experience Required!
- Must be highly organized, professional & motivated
- Should have solid computer skills
- Excellent communication skills
- Should be friendly and a team worker
- Will participate in the Nurse Management On Call Rotation
