Minimum Data Set (MDS) Coordinator (LPN) assesses long-term care patient health needs in collaboration with the Minimum Data Set (MDS) Coordinator (RN). Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Minimum Data Set (MDS) Coordinator (LPN)'s years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Job Duties
· completing minimum data set assessments
· creating comprehensive plans of care after review of the patient’s medical record
· communication with direct care staff, the patient’s physician, and family
· develops and completes the patient assessment process in accordance with the requirements of federal and state regulations and company policies and procedures.

Qualifications
· Must have experience in a skilled nursing and rehabilitation setting
· Must posse strong clinical skills
· Must have a strong understanding of the MDS process
· At least 1 year working in a skilled nursing facility setting.
EDUCATION:
Must possess, as a minimum, a Nursing Degree from an accredited school of nursing, college or university, RN preferred.
