Description

Danish Care Center is a 65-bed skilled nursing facility focused on the well-being and happiness of our residents. 

Danish Care Center is looking for a LVN with MDS experience to join our team.
 

Responsibilities of the MDS Nurse:

• Oversee utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's.
• Observe direct nursing care, review documentation, and report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the Nurse Supervisor 
• Plan, schedule and revise the MDS, including the implementation of RAPs, triggers, and care plans 
• Complete the entire MDS process is in a timely and consistent manner.
• Ensure the successful, timely electric transmission of the MDS package.

• Must have excellent leadership and communication skills;

• Must be a licensed LVN in the state of California;

• Must have skilled nursing experience;

• Must have basic MDS experience.
