Summary of Position
located at Salem Hospital ED
The Emergency Department Nurse Navigator is responsible for accurate and efficient patient identification for the MGB Home Hospital. The ED Nurse Navigator identifies eligible patients and conducts clinical and social eligibility and safety screening while collaborating with the Home Hospital Admitting provider, the Home Hospital Case Manager, and the Administrative Coordinator. This role also communicates with the ED care team, the patient and family and the ED Case Managers at times to facilitate transfers to MGB Home Hospital. This role is the patients' first introduction to MGB Home Hospital because of this, the role requires a strong understanding of the program and the ability to present the program accurately, addressing any questions raised by the patient, family or care team.
The ED Nurse Navigator is present in the ED, proactively, case-finding by engaging in rounds and huddles, as well as independently identifying patients throughout the shift to maintain goals of Home Hospital growth and census.
