Complete Health Care Services provides preventative health maintenance and acute condition management to adult and geriatric residents in long term care and rehab facilities in the Kansas City, MO metro area. We are a small practice currently employing 2 physicians and 7 nurse practitioners. Collaboration from attending providers is available 24/7 and we offer a competitive salary. The position of Geriatric Nurse Practitioner consists of providing Advanced Nursing Care and treatment to geriatric patients. It also consists of performing physical examinations, ordering diagnostic tests, developing treatment plans, and prescribing medications and/or therapies in collaboration with Physician.
These expectations include, but are not limited to:
Develop treatment plans based on scientific rationale, standards of care, and professional practice guidelines.
Diagnose and/or treatment of acute and chronic health care problems such as illnesses, infections and injuries.
Educate patients about self-management of acute or chronic illnesses, tailoring instructions to patients’ individual circumstances.
Order, perform, or interpret the results of diagnostic tests as appropriate.
Recommend interventions to modify behavior associated with health risks.
Appropriate care, treatment, and counseling for patients and their families.
Progress Notes, History & Physicals, and Discharge Summaries written on each patient on the rehabilitation unit Monday through Friday, and all charting completed within a timely manner in accordance with each facility Bylaws.
Consultations in acute care and skilled/assisted nursing facilities, and also follow up/subsequent visits on acute care and skilled/assisted nursing facility patients.
Provide facility-based services to patients in collaboration with Physician
Expectation of average of 20 (twenty) patient encounters per working day (after 90-day orientation period.
Orders and dispensing of written and/or electronic prescriptions as necessary in collaboration with Physician.
Respond to requests from facility staff, including but not limited to nurses, therapists, ancillary staff and patient families regarding patient care requests and issues that may arise.
