Nurse Practitioner - Ambulatory Care works in collaboration with physicians and other members of the healthcare team to manage care for patients in an ambulatory care setting. Orders, interprets, and records results of clinical tests and reports results to physicians. Being a Nurse Practitioner - Ambulatory Care prescribes medications and makes recommendations for other therapeutic forms of treatment. Assesses patient condition and may provide treatment using specialized medical equipment. Additionally, Nurse Practitioner - Ambulatory Care works in outpatient settings, responding to high patient volumes in short-term spans and dealing with unpredictable situations and issues. Follows established guidelines and protocols. Requires a master's degree. Requires Certified Registered Nurse Practitioner (CRNP). Requires Registered Nurse (RN). POSITION PURPOSE:
In collaboration with a physician, the Nurse Practitioner provides for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions for our transitional care/palliative care organization.
Transitional care is a combination of disease modification, primary care and management of distressful symptoms in individuals with severe illness—recently discharged from either an acute care facility (hospital), post-acute care facility (SNIF) or long-term care facility.
POSITION EDUCATION:
Master's Level Degree or MSN from an accredited program approved by the State Board of Nursing program.
POSITION EXPERIENCE:
Minimum of three-five years of clinical nursing experience. One-two years of Nurse Practitioner experience preferred.
CCU or ICU experience highly recommended.
