Job Responsibilities:

• Providing oversight and support to Case Managers
• Monitoring pediatric and adult patient care Managing referrals and assigning clinicians
• Coordinating care within the organization and with outside agencies
• Evaluating patient outcome scores related to value-based purchasing.
• Continuously assessing patient needs and using a team approach to assist patient and family in meeting those needs
• Developing, implementing, and updating plans of care

Requirements:

• A valid Colorado RN license in good standing
• Minimum of two years of management experience in home health, including OASIS experience
