The Care Coordinator is directly responsible for scheduling the home visits for the caregivers and assisting with the coordination of care. The Care Coordinator will maintain client relationships by assuring the skills of the caregiver match the needs of the client.

Responsibilities

• Conduct telephone intakes with new or potential clients
• Coordinate all scheduling activities, including replacement and substitute caregivers
• Resolve client, family and caregiver differences
• Maintain files to meet department standards
• Conduct initial home visits with prospective clients
• Finalize and invoice clients statements weekly
• Monday through Friday 8 am to 4 pm with occasional on call

Qualifications 

• Three years of scheduling experience in a home care environment.
• Articulate, with strong verbal and written skills
• Organized and detail oriented: good documentation skills
