Program Coordinator
We are looking for an experienced program coordinator to supervise up to 4 houses in the Cloquet, Esko and possibly Duluth area. Exceptional Person Centered Services and Behavioral needs desired. A successful program coordinator will need to be knowledgeable about 245 D rights and policies. The goal is to ensure all clients are supported to the best of the programs ability. Rotating schedule, typically M-F, hours have some flexibility. Work will be divided between homes and a small amount of office time in Cloquet.
This position will start out as hourly and will transition into Salary once training has been completed.
Responsibilities
· Supervise all Residential Program settings and the employees within the program
· Ensure supports are provided with dignity and respect to individuals served
· Duties are split between providing direct support, professional or program activities and supervision.
· Completing weekly updates to family, team members, and case managers on individuals status and progress towards goals.
· Following 245D licensing and documentation requirements including ISSA’s, IAPP’s, Treatment Plans, Person Centered Services, and required documentation.
· Working with the Program Director to ensure all licensing requirements and documents are completed.
· Ensure all safety and quality guidelines are maintained by staff.
· Implement person-centered service plans, provide guidance to staff on those ISPs and documentation.
· Ensure environment is kept clean and tidy, and schedule maintenance needs as necessary.
· Develop a budgeting plan for social events. Look for free events, plan outings, and develop budget guidelines.
· Completing face to face visits with all individuals at least weekly
· Assist with hiring and terminating employees.
· Completing appointment communication logs when clients attend appointments. Update treatment plans, and MARS as needed for changes.
· Attending all psychiatric and significant appointments with clients.
· Attending all client team meetings.
· Assist with planning and conducting monthly meetings
· Assist with rotating monthly documents from the house book to the clients master book
· Assist with scheduling client appointments and making referrals as needed.
· Monthly check: Fire Drill log (quarterly), Water Temperature Log (Monthly), Fire extinguisher log (quarterly), JV weight tracking (Monthly).
· Review client books: update, file, and organize as needed.
· Complete monthly schedule- Gathering schedules, requests, completing online scheduling and filling shifts (This does not mean covering all shifts that are not filled).
· Complete new employee training and ensuring all trainings are up to date annually.
· Assist with medication re-orders and ensuring all clients medications are in stock at the home.
· Review all employee training logs and ensure they are completed. Remind employees when annual trainings are due. Ensure we have all certificates ( Background study, CPR, VA, Med Ed)
· Provide staff with information for on-going trainings and educational options within the community.
· Review house menu’s to ensure individuals are being provided a healthy well balanced menu, and expenses are staying within budget.
· Review shift tasks occasionally to ensure all needs are being met.
· Review client and house money logs to ensure they are accurate and reporting errors.
· Reviewing all incident reports with staff and management for further action.
· Consistently review expenditures to ensure the agency is efficient: activities, meals, staffing, house needs.
· Bring complaints and concerns to supervision to Program Director to review.
· Assist with maintaining client benefits to ensure payments are received.
· Review client books occasionally to ensure all documentation is being complete.
· Working with Carlton County and DHS licensing to be sure all licensing needs are being met.
· Monitor program for efficiency, offer suggestions on improvements, changes, and needs.
· All other duties as assigned.
