Outpatient Care Coordinator supervises the day-to-day activities of the patient relations team of a healthcare organization to deliver patient liaison services and patient-centric care. Coaches staff on best practices for responding to patient and family questions, concerns, and issues. Being a Outpatient Care Coordinator facilitates collaboration with multi-disciplinary teams to make recommendations for improvements to the patient experience. Ensures that escalated problems are addressed and resolved. Additionally, Outpatient Care Coordinator provides and maintains up-to-date informational materials and resources for patients and families. Follows all applicable regulations regarding patient information privacy policies. May require an associate degree. Typically reports to a manager. Working team member that may validate or coordinate the work of others on a support team. Suggests improvements to process, is a knowledge resource for other team members. Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 327-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus as well as at 10 Eskenazi Health Center sites located throughout Indianapolis.
This is a grant funded role.
Adult Outpatient focuses on treating the whole person and helping clients integrate into society. Convenience is key for individuals struggling with mental health concerns and this program seeks to serve them close to home with community mental health centers. Services include case management, individual and group training for ADLs, group therapy, some limited individual therapy and individual sessions with certified peer recovery specialists, full psychiatric nursing services including medication training and monitoring, and evaluation. Adult Outpatient also partners with the Supported Employment Program (SEP) and Card Shop to help clients learn skills necessary to find employment and to remain successful with continued follow-up by the SEP consultants.
The Care Coordinator advocates for clients to ensure access to needed services which includes developing community relationships and resources. In collaboration with the MD/CNS and treatment team, the Care Coordinator formulates, develops, and revises treatment plans, BPSA's, ANSA and CANS assessments, and other necessary client assessments.
This role is in the community up to 50% of the time, with majority of services being homebased. The Care Coordinator creates their own community schedule, providing flexibility to focus each day by geographical areas. All clients are within Marion County, and we reimburse for mileage. The schedule for this role is Monday through Friday, 8:00am to 4:30pm.
Bachelor's degree required, with the ability to meet minimum competency standards set forth by Sandra Eskenazi Mental Health Center; some Sandra Eskenazi Mental Health Center programs may have additional certification and/or educational requirements
