Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, conducting assessments, and ensuring that patients receive the appropriate services throughout their treatment journey. This role requires a strong clinical background and excellent communication skills to effectively collaborate with healthcare providers, patients, and their families.

Duties

• Conduct comprehensive patient assessments to determine healthcare needs and develop individualized care plans.
• Coordinate outpatient services and manage transitions of care for patients moving from acute settings such as ER, ICU, or OR to home or other facilities.
• Monitor vital signs and evaluate patient progress, adjusting care plans as necessary.
• Collaborate with multidisciplinary teams to facilitate effective communication and ensure continuity of care.
• Advocate for patients by providing education on treatment options, resources, and support services available.
• Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with regulatory requirements.
• Engage in managed care initiatives to optimize patient outcomes while ensuring efficient use of resources.
• Provide support for patients with behavioral health needs and those requiring medical-surgical interventions.

Qualifications

• Current Registered Nurse (RN) license in the state of practice.
• Experience in PICU, ER, ICU, or outpatient settings preferred.
• Strong skills in patient assessment and critical thinking.
• Knowledge of managed care principles and practices is a plus.
• Ability to work collaboratively within a team-oriented environment.
• Excellent communication skills for effective interaction with patients and healthcare professionals.
• Familiarity with electronic health records (EHR) systems is advantageous.
• Commitment to ongoing professional development and staying current with nursing best practices.
