The Outreach Specialist will conduct outreach activities to re-engage People Living with HIV (PLWH) into care and treatment services. The Outreach Specialist will skillfully strategize innovative ways to locate patients, and seamlessly transition clients to Specialized Medical Case Management (SMCM) The focus for the Outreach Specialist will be to ensure individuals who have fallen, or are falling, out of care are contacted and ultimately re-engaged into care. The Outreach Specialist will spend a high percentage of time outside of the office conducting outreach activities.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
1. Provide knowledge to clients on what services are offered within the agency.
2. Locates and contacts clients with a compassionate approach.
3. Interview clients using the Return to Care Assessment (RTC) to identify barriers to
4. continuous engagement in care and treatment.
5. Serves as part of the client’s support system at the beginning of the SMCM intervention.
6. In coordination with SMCM, helps develop clients’ enhanced care plans.
7. Manages a cohort of clients and systems to discreetly contact clients.
8. Manages reports that are updated daily to review Outreach progress.
9. Ensures that Protected Health Information (PHI) is kept confidential, in compliance with
federal, state, and discipline-specific laws and regulations.
10. Able to work flexible work hours to support systems of contacting clients.
11. Able to travel within the service area 50% or more each week.
12. Other Duties as assigned.
Minimum Qualifications (Knowledge, Skills, and Abilities)
· Must maintain a valid S.C. driver’s license or valid commercial driver’s license with satisfactory driving record per Palmetto Community Care Standards.
· Excellent interpersonal and communication skills, both orally and in writing.
· Quality customer service skills are a must.
· Must be organized, professional, self-motivated, and able to effectively work in a team environment.
· Professional, comfortable, and adept at providing non-judgmental customer service to diverse individuals.
· Ability to interact well with people and provide an atmosphere of trust, mutual respect and caring.
· Experience with Excel spreadsheets, Word documents and other Office applications.
· Strong and timely documentation skills.
· Ability to effectively present information and respond to questions from clients, managers, and employees using tact, courtesy, and cooperativeness.
· Prior knowledge of HIV and STI trends preferred but not required.
· Knowledge of Charleston, Berkeley, and Dorchester County areas required.
