POSITION SUMMARY:
As the Patient Services Coordinator, you will be responsible for scheduling the home visits for the field staff and assisting with the coordination of patient care. You will work under the direction of the Clinical Manager to ensure proper utilization of clinical resources to maximize care center capacity and utilization guidelines.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Coordinate with recruiting team, Area Director and HHAC to identify staffing pool.
- Review daily, weekly, and monthly open and unassigned shifts to fill.
- Schedule intake assessments.
- Schedule patients for the whole certification period for HHA and nursing services once onboarded.
- Ensure missed visits are scheduled by end of week or flagged to Clinical Manager/Area Director for resolution.
- Prepare clinician schedules and optimizes for geography, caregiver/patient needs, etc.
- Monitor hospitalized patients and assists with transfers.
- Other duties as assigned.
- High School diploma or GED equivalent.
