Description
We are dedicated to providing quality and compassionate care in our service areas and are currently offering a sign-on bonus of \(1500 for Medical Assistants and an additional \)2500 for Nurses. As a Patient Care Specialist, you will be part of a person-centered integrated care team, offering case management support to patients without insurance or with commercial insurance receiving services in a primary care setting.
Responsibilities
- Maintain the Referral for Testing/Procedures/Referral List
- Gather pertinent information from healthcare professionals to complete referrals/pre-certifications/prior authorizations.
- Contact various offices/facilities to obtain necessary information for accurate referral requests.
- Prepare and process all referral paperwork, including pre-certification and prior authorization requests.
- Work with patients to schedule tests/procedures and follow up to ensure appointments are completed.
- Respond to patient inquiries regarding health plan referrals.
- Assist patients in enrolling in Patient Assistance Programs for prescribed medications.
- Maintain inventory of Patient Assistance Program medications and log information.
- Complete call backs to patients regarding their test results as per directives.
- Ensure timely and accurate documentation of all support services provided.
- Advocate for culturally competent services to meet the unique cultural needs of each patient.
- Collaborate with Behavioral Health Services Department and/or Community Support Workers as applicable.
- Maintain an accurate referral database.
- Assist with data collection for the annual Uniform Data System (UDS) report.
- Attend all required trainings and staff meetings.
- Complete prior authorization and documentation for medications and Durable Medical Equipment (DME).
- Obtain results from referrals and diagnostics, ensuring appropriate Electronic Health Record charting.
- Provide information related to community resources specific to patient needs.
- Perform other duties as assigned.
Qualifications
- High School diploma or GED required; Associate's or Bachelor's in nursing preferred.
- At least two years of equivalent work experience required if an Associate’s degree or medical assistant certification is not obtained.
- Minimum of two years medical office experience required.
- Social work and behavioral health knowledge experience preferred.
- Proficiency in Microsoft Office Suite required.
- Ability to maintain confidentiality is essential.
Licensure/Certification
- Medical Assistant Certification required if less than two years of college or equivalent work experience is not obtained; Licensed Practical Nurse (LPN) preferred.
