Provides consultation and answers inquires from patients, families, physicians and other healthcare professionals regarding drugs and their use. Being a Pharmacist - Home Care maintains all required documentation and data on prescriptions, inventory, and patients. Adheres to JCAHO standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Pharmacist - Home Care's years of experience requirement may be unspecified. Busy home care agency looking for Registered Nurses to case manage our patients in their homes, in 3 areas. We are looking for a nurse for each area; Brockton, New Bedford, and Fall River. We offer 401(k), competitive per-visit pay rates, paid time off, flexible schedules, and health/dental insurance.
We are looking for someone with the ability to use our iPads to complete their visits at the patients' homes, call and follow up with doctor's offices, provide detailed reporting to other clinician's on their team, and keep the clinical manager updated.
We are looking for a responsible person that is able to submit their paperwork on time and is able to work on their own, without direction. We do provide training. You will be given an iPad with case and charger, a nursing bag with equipment and medical supplies, and will be trained on our systems and processes.
