Job Summary: This is a full-time position responsible for providing specialty pharmaceuticals and therapy management support services to obtain optimal therapeutic outcomes thus avoiding adverse events and unnecessary costs. The pharmacist in charge will provide oversight of safety, appropriateness, and effectiveness of pharmaceutical care. Must be a registered pharmacist in the state of TEXAS and has demonstrated sufficient education and experience in the products and services that The START Center Pharmacy provides.
Responsibilities include, but not limited to:
1. Presents a polite, considerate, and professional manner.
2. Offers consistent patient management, provides patient support, and focus on patient adherence to therapy.
3. Provides patients education and counseling to promote enhanced patient understanding, increased adherence to medication regimens and prevention and detection of adverse drug events and patterns of over-use and under-use of prescription medications.
4. Conducts a limited but appropriate needs assessment of patients.
5. Conducts a comprehensive medication review.
6. Provides and records care plans for medications and/or treatment regimens to our patients.
7. Process and Maintain the positive quality interventions.
8. Educate pharmacy staff and other providers on all new and any updates on oncology / hematology medications.
9. Assist Pharmacist in pharmacy as needed as well as cover during times of absence.
10. Reviews and assesses prescription orders; compounds (non-sterile) and dispenses medications. Assesses patient’s response to drug therapy and assists with patient care activities.
11. Processes purchases/returns and inventory of pharmaceutical agents and products; assists with patient assistance. Ensures the integrity of the medication supply.
12. Provides drug information to patients, providers, nurses, and staff. Counsels patients regarding medications. Provides education for providers, nurses, technicians, and staff.
13. Directs, assists, and resolves problems encountered within the medically integrated retail pharmacy department.
14. Assists the pharmacy technicians in the prior authorization process, patient assistance process, insurance verification when appropriate.
15. Enhance patient compliance to oral therapies. Collaborate with patients and their physician team to minimize side effects and increase adherence.
16. Complies with The START Center Pharmacy and department policies and procedures.
17. Ensures quality improvement and maintains appropriate standards.
18. Maintains professional growth and development, participates in professional affiliations, and reviewing applicable, current literature.
19. Complies with all REMS guidelines and ensures pharmacy technicians follow the current guidelines.
20. Supervises pharmacy technicians within the retail pharmacy dispensing area.
21. Ensures applicable continuing education and licensure are maintained.
