Establishes individualized treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment. Being a Physical Therapist - Home Care maintains patient records by keeping track of goals and progress while adapting treatment plans accordingly. Advises patients and their families on in-home treatment options and what to expect from the recovery process. Additionally, Physical Therapist - Home Care may work with and/or consult with physicians, case managers, and other medical professionals. Requires a doctorate degree in physical therapy. Job Summary: The Physical Therapist provides evaluation and treatment services to the patient in the home setting. Directs and assists patients in exercises, muscle re-education, ADL's, gait and transfer training and use of PT modalities, establish/upgrade home exercise program, and coordination and balance activities. Member of the InterDisciplinary team. Is responsible for supervising certified staff (PTAs, HHAs). Is responsible for instructing Home Care staff members, and family members to assist with the patients therapy program. Reports to the Home Care Director and/or the Patient Care Coordinator.
