Job summary:

• Provide care to patients in the privacy of their home setting
• Working through admissions and routine visits to develop and carry out care plans
• Participating in interdisciplinary meetings to collaborate on approaches to patient care

Qualifications:

• Licensed to practice as a physical therapist (PT) within Oregon state (or before your first day of work).
• One to two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
• Possesses and maintains current CPR certification.
