Physical Therapist - Home Care jobs in Newhall, CA

Physical Therapist - Home Care evaluates and treats patients recovering from injury, disease, or surgery in a home care environment. Establishes individualized treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment. Being a Physical Therapist - Home Care maintains patient records by keeping track of goals and progress while adapting treatment plans accordingly. Advises patients and their families on in-home treatment options and what to expect from the recovery process. Additionally, Physical Therapist - Home Care may work with and/or consult with physicians, case managers, and other medical professionals. Requires a doctorate degree in physical therapy. Requires a state license to practice physical therapy. Typically reports to a manager. Physical Therapist - Home Care's years of experience requirement may be unspecified.   • A Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Supervisor or Manager.

    ● $90.00 Physical Therapy Re-evaluation
    ● $90.00 Physical Therapy Routine Visit
    ● $120.00 Physical Therapy Start of Care

    PRINCIPAL DUTIES AND RESPONSIBILITIES (Included but not limited to):

    • Understands and adheres to established policies and procedures.
    • Adheres to local State, Federal and Accreditation regulations
    • Provides physician prescribed physical therapy.
    • Improves or minimizes residual physical disabilities of the patient.
    • Returns the individual to optimum and productive level within the patient’s capability.
    • Participates with all other home care personnel in patient care planning.
    • Directs and supervises personnel as required.
    • Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.
    • Performs all skilled procedures as ordered by the physician.
    • Consults with physicians regarding change in treatment.
    • Notifies the Director of Patient Care Services/ Clinical Supervisor/ Manager on any changes in patient's medical condition.
    • Completes all documentation in the established electronic medical record system timely and accurately.
    • Writes reports to physicians regarding the patient's progress.
    • Instructs patents and family / significant others in home programs and activities of daily living.
    • Participates in case conferences, in-services and educational sessions.
    • Participates in the QAPI activities as assigned by the Director of Patient Care Services.
    • Completes and submits the OASIS assessments, reassessments, transfers, discharges resumptions of care, and transfers and significant change in condition in accordance with Agency defined time frames.
    • Consults with family and Agency personnel.
    • Prepares clinical and progress notes.
    • Able to maintain strict confidentiality with regard to protected and sensitive information.
    • Assigned other job duties within the physical therapy scope of practice.


    EDUCATION, EXPERIENCE and LICENSES:

    • Graduate from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the AMerican Medical Association, or the Council of Medical Education of the American Medical Association and AMerican Physical Therapy Association.  The Physical Therapist must have a Bachelor’s, Master’s, or Doctorate degree in Physical Therapy
    • Currently licensed in the state in which practicing
    • Two (2) years’ experience preferred in a home health setting
    • Knowledgeable and experience with Medicare OASIS documentation

    SKILLS AND ABILITIES:

    • Possesses keen knowledge of medical terminology
    • Excellent Communication skills; ability to be flexible and work well with others.
    • Ability to maintain confidential information.
    • Ability to prioritize and multitask.
