Qualifications: Certified by the American Board of Family Practice or American Osteopathic Association of Family Practice, graduate of accredited medical or osteopathic school of medicine and currently licensed to practice in the State of Ohio. Completion of a three-year family practice residency program or active hospital-related practice for at least seven years required.
