Job Summary:
- Coordinates team-based care to provide health services to individuals, through effective partnerships with patients, their caregivers,/families, community resources, and their physician.
- Facilities "shared goal model" within and across settings to achieve coordinated high-quality care that is patient - and family-centered.
- Ensures continuity and coordination of care across transitions; identifies barriers and facilitates optimal care delivery, and develops a comprehensive plan to address needs including prevention, self-management, access to community resources and education.
Qualifications:
- Associates degree required. Bachelors of Nursing preferred.

Magna (/ˈmæɡnə/ MAG-nə) is a census-designated place (CDP) and township in Salt Lake County, Utah, United States. The population was 26,505 at the 2010 census, a moderate increase over the 2000 figure of 22,770.
