    HOURS: Monday – Friday, 37.5 hours per week.


    POSITION SUMMARY:

    Responsible for providing a quality program for prenatal women and families with emphasis on the social-emotional and cognitive development of infants and toddlers. Responsible for providing a quality home visiting experience for each participating family that provides opportunities designed to enhance the parent-child relationship, promote the development of parenting skills, and foster an environment in which the child can develop to his or her full potential. Responsible for planning group activities and educational opportunities for parents. Facilitates parent participation in the development of the Family Partnership Agreement and provides support and referrals to other community agencies.


    PROGRAM RESPONSIBILITES:

    • Provides weekly home visits for home-based families, regular visits/contacts for center-based families and prenatal women, scheduling all contacts at the convenience of the family.
    • Ensures that annual required home visit numbers are maintained by scheduling home visits for families with the Health Services Coordinator as needed and that all visits are documented on the Family Contact Record and in ChildPlus.
    • Plans developmentally appropriate curriculum in collaboration with parent during weekly home visits for parent to engage child in activities designed to enhance the parent/child relationship and promote the child’s growth and development. Uses the EHS Home Visit Plan as a tool to plan, record and document curriculum, in conjunction with the Head Start Early Learning Outcomes Framework.
    • Plans activities and educational opportunities for prenatal women during home visits with the goal of a healthy pregnancy outcome with the use of PAT and supplementation with the Partners for a Healthy Baby Curriculum.
    • Completes Family Interests and Needs Survey with families at or near enrollment, and again mid-year. Follows up with requested information and referrals resources.
    • Facilitates parents’ participation in the development of Family Partnership Agreement. Updates with family on an ongoing basis.
    • Responsible for keeping required records in accordance with the program’s health requirements; physical examinations, vision, hearing, dental screenings, dental exams and all necessary follow-up in these areas or other health needs. Documents progress and completion of events in ChildPlus.
    • Provides support to EHS/HS families through information, education and referrals to appropriate resources. Provides follow-up as needed.

    • Communicate proactively with the supervisor regarding workflow, problems, suggestions, etc.
    • Contribute to a positive, team-oriented work environment; and attend team and staff meetings as scheduled.
    • Be punctual for scheduled work, use time appropriately, and complete work in a timely fashion with minimal errors.

    Requirements

    MINIMUM QUALIFICATIONS:


    Experience:

    • Knowledge and ability to work with community human service providers and related agencies
    • Ability to effectively communicate and relate comfortably to young children and families.
    • Must have basic computer literacy skills.
    • Fluency in language spoken primarily by the majority of EHS families.

    Education:

    • Home Visitor Child Development Associate (CDA) credential.
    • Associate’s or Bachelor’s degree in Early Childhood Education, Child Development, Social Services or related field preferred.
    • Possess or be enrolled in Parent as Teachers (PAT) foundational training.

    License/Certification:

    • Successful completion of criminal record check.
    • Valid driver’s license with access to reliable transportation for required travel.
