Qualifications

Degree requirements:

• Licensed by the NYS Education Department as Master Social Worker (LMSW), Clinical Social Worker (LCSW), Registered Professional Nurse (RN), Physical Therapist, Speech Language Pathologist, or Occupational Therapist
• Master of Social Work degree
• Doctorate or Master of Psychology degree
• Certified Teacher of Students with Disabilities
• Certified Rehabilitation Counselor

AND

One year of experience developing and implementing day habilitation plans; providing vocational education services; providing residential habilitation services; or providing job coaching or supportive employment services.

OR

Individual with a Bachelor’s degree in related field and two (2) years of experience developing and implementing day habilitation plans; providing vocational education services; providing residential habilitation services; or providing job coaching or supportive employment services.

Other:

• At least three years of supervisory experience required
• Excellent communication skills

Job Responsibilities

• Oversee and manage Structured Day Program (SDP) services to ensure they are individually designed services, provided in an outpatient congregate setting or the community, that work to improve or maintain the participant’s skills and ability to live as independently as possible in the community.
• Ensure compliance with all regulatory and waiver requirements.
• Supervise all SDP staff, making sure that they are following all guidelines as outlined in their job description.
• Ensure that all educational, vocational, and social goals (as outlined in each participant’s service plan) are being implemented appropriately.
• Plan and implement regular community outings.
• Ensure that the program environment is maintained in a safe and clean manner.
• Manage and ensure that all billing is completed accurately and on time.
• Ensure that participants’ ISRs and goal plans are up-to-date and that each participant is working on the goals as outlined in the plans.

Pay: $21.00 - $24.00 per hour
