  • JOB SUMMARY
     
    The Quality Improvement Coordinator works with executive leadership to plan, organize, direct, and lead the work process of the quality improvement program. Scope of responsibly can encompass all quality improvement settings of the organization but is primarily clinically-focused. The coordinator will have intimate familiarity with the corporate strategic plan, clinical grant work, HRSA directed goals and payer incentive programming.  The coordinator has detailed understanding and can skillfully use the techniques of system design, project management, quality improvement, outcomes measurement, and statistical analysis. The coordinator is responsible for conformance to regulatory requirements and contractual obligations as designated to the organization's quality improvement program.
     
    ESSENTIAL FUNCTIONS
     
    1. Implement the organization’s Quality Improvement Plan initiatives.
    2. Establish and measure quality indicators monthly or as needed to meet requirements of grants or federal funding.
    3. Review and evaluate patient medical records as needed to assess clinical quality indicators.
    4. Develop and utilize electronic method of data collection and storage.
    5. Target low-performing indicators with improvement strategies and track changes over time to ensure strategies have been effective.
    6. Work with the provider departments on departmental Quality Improvement projects.
    7. Prioritize targeted areas for improvement and develop a timeline to address them.
    8. Develop and lead cross-functional teams in the development and implementation of improvement strategies as the leader of the CQC committee.
    9. Collaborate with other agency staff in areas of Quality Improvement.
    10. Assist in building a culture of quality within the agency.
    11. Educate staff on Quality Improvement concepts and tools.
    12. Engage staff in participation in QI activities.
    13. Assist with updating and revising policies and procedures as needed in support of quality initiatives.
    14. Support and assist the CMO in preparation of HRSA/other government reporting and certifications.
    15. Interface with payer representatives related to work surrounding shared savings or incentives.
    16. Actively participate as Quality representative to WPHCA and other agencies in the State.
    17. Interpret and input data as required for payer programming.
    18. Perform other duties as assigned.
    EXPERIENCE/SKILLS
     
    1. Experience in Clinical Quality Improvement of 3-5 years. Familiarity with CMS – 5 Stars, HEDIS, NCQA and/or PCMH helpful.
    2. Bachelor’s Degree.
    3. Clinical background, a plus.
    4. Demonstrate initiative, ability to work with others, and good professional judgment.
    5. Ability to work independently and organize time effectively.
    6. Excellent written and verbal communication skills.
    7. Strong organizational skills and attention to detail.
    8. Knowledge of and well-skilled in Microsoft Word, Excel, Outlook, Vizio and Power Point.
    9. Coaching, mentoring, and teaching skills.
    10. Ability to relate well to people from diverse ethnic and cultural backgrounds.
