ESSENTIAL DUTIES AND RESPONSIBILITIES
Client Assessment and Care Planning:
• Conduct initial client assessments to determine their specific needs and preferences and Caregiver Intro.
• Develop individualized care plans in collaboration with clients and their families.
• Monitor and update care plans as needed based on changing client requirements.
Staff Management:
• Conduct performance evaluations and provide feedback to staff.
• Address staff concerns and issues promptly.
Quality Assurance:
• Ensure that all caregivers follow agency policies and procedures.
• Conduct regular quality checks and home visits to assess the quality of care provided.
• Implement corrective actions and improvements when necessary.
Client Safety:
• Promote a safe and comfortable living environment for clients.
• Monitor client safety and implement safety measures as required.
• Educate caregivers on safety protocols.
Documentation and Reporting:
• Enter care plans and assessments into WellSky
• Maintain accurate records of client care, including daily logs and incident reports.
Training and Education:
• Ensure caregivers attend ongoing training and education on best practices, policies, and procedures.
• Ensure caregivers are trained in handling emergency situations.
Additional Responsibilities:
• Provide support and backup to scheduler as needed.
• Provide support as a caregiver in emergency situations.
• Participate in on-call rotation.
• Obtain client and caregiver photos during home visits.

PHYSICAL REQUIREMENTS:
Able to lift 50 lbs. using safe, ergonomic movements
Able to stand, walk, reach, crouch, use hands and fingers
