We are operating in the North Scottsdale area, and will expand, grow partnerships, and provide our service to seniors throughout Phoenix.

Senior care is a high growth industry, and Home Matters Caregiving is focused on being at the forefront of that growth by prioritizing our team members and clients. The Care Supervisor will primarily focus on ensuring the well-being and quality of care provided to clients in their homes.  

ESSENTIAL DUTIES AND RESPONSIBILITIES

Client Assessment and Care Planning:

• Conduct initial client assessments to determine their specific needs and preferences and Caregiver Intro.

• Develop individualized care plans in collaboration with clients and their families.

• Monitor and update care plans as needed based on changing client requirements.

Staff Management:

• Conduct performance evaluations and provide feedback to staff.

• Address staff concerns and issues promptly.

Quality Assurance:

• Ensure that all caregivers follow agency policies and procedures.

• Conduct regular quality checks and home visits to assess the quality of care provided.

• Implement corrective actions and improvements when necessary.

Client Safety:

• Promote a safe and comfortable living environment for clients.

• Monitor client safety and implement safety measures as required.

• Educate caregivers on safety protocols.

Documentation and Reporting:

• Enter care plans and assessments into WellSky

• Maintain accurate records of client care, including daily logs and incident reports.

Training and Education:

• Ensure caregivers attend ongoing training and education on best practices, policies, and procedures.

• Ensure caregivers are trained in handling emergency situations.

Additional Responsibilities: 

• Provide support and backup to scheduler as needed.

• Provide support as a caregiver in emergency situations.

• Participate in on-call rotation.

• Obtain client and caregiver photos during home visits.

WORKING ENVIRONMENT:

      Office - Weekdays

   Client Residence - Assesments/Supervisory Visits/As Needed

PHYSICAL REQUIREMENTS:

      Able to lift 50 lbs. using safe, ergonomic movements

      Able to stand, walk, reach, crouch, use hands and fingers

      Able to talk, hear and see

                Physically and mentally able to drive a vehicle
