    Your Role as a Case Manager 2:

    This role is suitable for individuals who have some experience in case management and are looking to further develop their skills and knowledge within our specialized population and scope.

    As a Case Manager, you will:

    • Provide Client Care (65-75% of time): Conduct assessments, offer basic needs and connect clients with essential resources. You'll create client centered goal plans to support their needs develop life skills to help them thrive.
    • Document and Report (20% of time): Maintain accurate case notes, track client progress, and ensure data integrity.
    • Engage in Community Outreach: Raise awareness of homeless youth issues, represent our agency in community meetings, and foster strong partnerships.
    • Ongoing Training & Development: Complete required training and actively work towards Community Health Worker and/or QMHA certification to enhance your expertise.

    What We're Looking For:

    • Education: High School Diploma, GED, or College Degree (2 or 4 years). We recognize the value of formal education but also consider relevant life experiences and ongoing learning. Equivalent life experiences or specialized training relevant to the role will also be considered.
    • Experience: A minimum of three years of direct experience in social services required, particularly with youth and/or families. This can include paid employment, internships, volunteer work, or relevant personal experiences.
    • Additional Experience: At least two years of paid case management experience in a relevant field (with Maslow Project or elsewhere).
    • Demonstrated Application of Best Practices: Candidates should show emerging and developing evidence of understanding and application of learned best practices, such as Self-Sufficiency, Trauma-Informed Care, Positive Youth Development, and Fostering Resiliency. This can be demonstrated through previous work experiences, training, or volunteer involvement.
    • Eligible to be certified as a Community Health Worker (CHW) through documented prior supervised hours and/or ability to reinstate a lapsed certification. Able to begin QMHA-1 certification if assigned.
    • Employee Expectations: Willingness to collaborate and maintain positive relationships with clients, team members, and community partners. Evidence of professional communication, problem-solving, and boundaries as well as usual workplace expectations.

    Physical and Working Conditions:

    This role involves a mix of office work, community engagement, and occasional outdoor activities in various weather conditions. Physical demands include walking, standing, lifting, and occasional driving.
