TITLE: ASSESSOR (Part Time)
SUPERVISOR: EXECUTIVE DIRECTOR/TEAM LEAD SUPPORT
Hours Per Week: 20 hours
East Central Kansas Area Agency on Aging serves older adults through case management, information and assistance, and meals in six counties and provides assessment and service provider oversight to intellectually and developmentally desabled populations in three counties. We provide a variety of services and resources to help educate and to help keep individuals at home in their community as long as possible. Assessments are required for case managed services and waivered programs per state and federal program requirements. This position is a blend of assessment responsibilities for those who only require an assessment through the waivered programs, who need assessed for eligibility for appropriate long-term care placement, and those who need in-home services. Agency vehicles are provided for travel to complete the responsibilities of the position.
Salary based on experience and will be reveiwed based on performance.
RESPONSIBILITIES:
Administer state required assessments (FAI, BASIS, CARE, UAI, OTHER). Assessmsents are distributed by geographic area within the six counties.
Develop and provide appropriate information, referrals and resources as appropriate to meet needs of older adults and act as advocate for their long term needs.
Coordinate with multiple agencies and service providers. Work with older adults, their families and caregivers to provide necessary services.
Other duties as assigned required to provide agency services.

QUALIFICATIONS:
Four year college degree is required. Preferred emphasis in aging, social services, family studies, psychology, social work, or human development but we will train the best candidate for the job.
Ability to access a variety of facilities and homes to meet with clients.
Computer experience with Word and Excel.
Valid Driver’s License and clean Motor Vehicle Report.

Pay: $21.00 - $24.00 per hour
