Six Sigma Quality Program Director plans, designs, and directs Six Sigma projects. Develops strategy and policies required to accomplish measurable business process improvements. Being a Six Sigma Quality Program Director employs Six Sigma/Lean methodology and analytics to re-engineer organizational operations. Works with other certified professionals in the planning, designing, and implementation of key projects. Additionally, Six Sigma Quality Program Director coaches and trains teams in the utilization and deployment of Six Sigma tools and principles. Requires Six Sigma certification as Black Belt. May require a master's degree. Typically reports to top management. The Six Sigma Quality Program Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure.   • Epic Nursing Services is seeking to employ a program care manager to provide clinical assessments, clinical evaluations, psychosocial counseling assistance, and supervision of all members and personnel involved in the GAFC/ AFC program.

    Qualifications.

    • A bachelor’s degree, a social worker license from the Massachusetts Board of Registration in Social Work, and at least two years of recent experience working with elders or adults with Disabilities is preferred or
    • A bachelor’s degree and two years of clinical experience in the care of elderly or people with disabilities is preferred.

    Responsibilities.

    • Complete and coordinate all applicable; clinical assessments, clinical evaluations, psychosocial counseling assistance, and supervision of personnel and members of the AFC/GAFC program.
    • Develops and review on an ongoing basis each member’s AFC/GAFC plan of care; and review the PCP Summary Form and Provider order form.
    • Monitor the health status of all members to ensure that all needed AFC/GAFC is properly delivered.
    • Report changes in the health status of any member to the member’s PCP.
    • Receives and acts to resolve complaints subject to the agency’s grievance procedure in the capacity of “next higher-level authority”. Apprises Administrator of potential legal risks
    • Conduct on-site visits with each member at the qualified setting per the visit cycles.
    • Complete a care management progress note corresponding with each on-site visit or encounter, or more often as the member’s condition warrants.
    • Oversees the clinical management of the Agency’s clinical department.
    • Assist the Administrator in planning the overall development and administration of the Agency as set forth in the policies and procedures.
    • Participates in the development of administrative policies and procedures relating to the Agency
    • Directs implementation of approved work methods and procedures that reflect elements essential to rendering high quality care
    • Participates in the review/analysis/appraisal of the effectiveness of the total Agency program
    • Utilizes the QA data reports to quantify Agency’s homecare performance.
    • Plan for and implementing discharges and transition from the AFC/GAFC program.
    • Conduct an orientation for each AFC/GAFC Direct Care Aide before the Direct Care Aide begins personal care; and provide ongoing training to Direct Care Aides and AFC Caregivers on health and aging as needed.
    • Obtaining a social history and conducting a psychosocial evaluation
    • Participating in the development, implementation, and ongoing review of the AFC/GAFC plan of care
    • Conducting an initial review of the suitability of the member’s home, and conducting a review at least annually, or more often as needed, of the suitability of the member’s home.
    • Maintain all staff and contract personnel files on personnel evaluations and make copies monthly for the appropriate department supervisor and keep all evaluation forms current.
    • Works closely with the recruiter in achieving satisfactory staffing leadership in promoting and maintaining standards for giving high quality care
    • Answering inquiries of general nature from applicants, visitors, and professional staff, assisting them in a friendly and cooperative manner as needed.
    • Assume various duties as directed by the Administrator.

    Job Type: Full-time

    Pay: $70,000.00 - $80,000.00 per year
