Provide moderate-to-intense community-based case management services for individuals with serious mental illness to promote community functioning and decrease psychiatric hospitalizations and crisis episodes.

Essential Functions:- Provide and coordinate a continuum of treatment services.
- Carry a caseload of clients providing supportive counseling and concrete services as assigned by Supervisor and operate as part of a treatment team.
- Ensure all documentation is completed in a timely manner and in accordance with Rule 140 requirements, DHS, and other funding/regulating bodies.
- Transportation of clients in personal vehicle and agency vehicle is required. Services will be provided in multiple locations.
- Other duties assigned.
Position Qualifications:- Bachelor's degree required, Master's preferred.
- 1-3 years of clinical experience preferred.
- Experience in Crisis Intervention and De-escalation.
- Excellent written and verbal skills.
- Valid Illinois Driver's License & Insurance.
