    Job Description:

    The primary purpose of your job position is to plan, organize, develop, and direct the overall operation of our facility’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

    Essential Duties (include but are not limited to):

    • Works with the patient, family/significant others and other team members to outline goals of stay at admission, the plan to meet those goals and discharge as appropriate.
    • Assists patient/family/significant others in obtaining appropriate forms to gain services, such as insurance appeals.
    • Advocates for the patients by helping them to navigate the length of stay for skilled services
    • May act as a resource specialist in a particular area as appropriate
    • Obtains and assesses information provided by patient and other provider referral sources
    • Works collaboratively with patients and their families toward meeting the need or resolution of the bio-psycho-social issues identified
    • Documents assessment and intervention plan in patients’ record
    • Educates patients and their families on various programs and/or health-related issues
    • Assists patients to order recommended DMEs as long as covered by patient’s insurance prior discharge.
    • Assists patients to schedule PCP follow up appointments prior discharge.
    • Schedule IDT initial/safe discharge plan care conferences to patient family/significant others
    • Sending required initial clinical notes/updated clinical notes to patient’s HMO insurance assigned Case Manager on requested review date
    • Interprets rules, regulations, policies and programs for patient and their families
    • Communicate with patients, families, health providers, insurance case manager
    • Performs related work as required

    Qualifications:

    Proof of COVID-19 primary vaccine series is required for all facility employees, (requests for accommodation due to medical exemption or religious belief will be reviewed)

    Must be able to read, write, speak, and understand the English language.
    Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
    Must be a supportive team member, contribute to and be an example of team work and team concept.
    Must be knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.
    Must possess the ability to make independent decisions when circumstances warrant such action.
    Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
    Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
    Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning.
    Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services.
    Must not pose a direct threat to the health or safety of other individuals in the workplace.
